Triage and Care Coordination
This training is designed to strengthen school-based mental health support systems by improving triage and care coordination. Participants will gain a clear understanding of the Multi-Tiered System of Supports (MTSS) framework and learn how to:
- Understand the MTSS framework for mental health support
- Develop skills in triage and prioritization of mental health referrals
- Enhance confidence in the implementation of effective care coordination
The session offers practical tools to support students at:
- Tier 1: Universal supports for all students.
- Tier 2: Targeted supports for groups or individuals with emerging needs.
- Tier 3: Intensive, individualized supports for high-risk students.
Through real-world scenarios, participants will build confidence in decision-making, foster collaboration with families, teachers, and community resources, and ensure continuity of care to meet students’ changing mental health needs.
