Community care coordination
No-cost, statewide benefit available to eligible Medi-Cal members that provides Enhanced Care Management-ECM. Gives members a single care manager to help coordinate care among multiple services (physical, behavioral, dental, developmental, social services). Making it easier to get the right care, at the right time, at home, in the community, or in a doctor’s office.

Who Qualifies for ECM Services?
Children, Youth, and Families may qualify for ECM Services through WYS if they:
- Are experiencing homelessness or unstable housing
- Have frequent visits to the emergency department, hospital, or short-term care
- Have serious mental health or substance use disorder needs
- Are involved in the child welfare system
- Are insured with a participating managed care plan
